Registration fees are to be paid by Monday 30th June 2026. Our preferred payment method is PayPal as part of completing this registration form. Alternatively, payment can be made by either:

This information has been collected for the primary purpose of Creative Kids and will be used to contact you regarding the event. This information may also be used to inform you of future children and youth activities being run by Willoughby & Northbridge Uniting Church. Following the current event, you may opt-out of receiving further notifications and information.

Children will participate in one elective for 1 hour each morning, and one elective for 1 hour each afternoon. The electives for each child will remain the same across the week (i.e. they don't change each day). Each child can nominate a first and second preference for both morning and afternoon electives.

Please be aware that some of our electives have limited places available. This is due to the use of small spaces (eg kitchens) or safety supervision ratios. Places are allocated as registrations are received. Where a child's first preference is no longer available, we will endeavour to assign the second preference.

However, we are not able to guarantee preferences, and reserve the right to assign electives to optimise the smooth running of Creative Kids (for example, to even out the number of participants across the electives).

I authorise that the child receive medical attention as required and treatment by ambulance/medical personnel if necessary. I acknowledge that every attempt will be made to contact me or alternative emergency contacts in the event that emergency medical attention is required. 

I consent to the child taking part in the approved program of activities for the Creative Kids Holiday Program. I appreciate that every care will be taken by the leaders, and that those connected with Creative Kids cannot be held responsible for personal injury, loss or theft of property affecting my child. 

During Creative Kids, activities will be filmed and photographed. I understand the images may be displayed in church publications, website and on social media. Children's names will not be published or linked with any footage. I consent to images of the child being used as part of Creative Kids materials.
